Dorothy Ann Conrad: "Dorothy Ann Conrad, 87, of Conway, died Sunday, September 17, 2017. She was born March 3, 1930, in Altoona, Pa., to the late Wasyl and Helen States Cramer.

She was a member of Holy Ghost Orthodox Church where she was a member of the Ladies Altar Society. Her passions in life were crafting, cooking, baking, traveling, and working in her garden. She was a devoted and loving wife, mother and grandmother.

Preceding her in death, in addition to her parents, were her husband, George Conrad, Jr.; son, Glen Conrad; and a brother, William Cramer.

Ok here is my simple flex builder contact form. It will validate fields then post form data to a backend page for processing.
